How to Complete the Replacement Form

When filling out the car registration replacement form, you must be ready to write down your personal information, as well as details pertaining to your vehicle. For example, you will need to include your name, residential address, daytime phone number and an email you can be contacted at.

Concerning your vehicle information, you will need to make sure you note your vehicle plate number and your Vehicle Identification Number (VIN) for identifying purposes. Before you can turn in your completed form, however,  you must include your signature at the bottom and indicate which document you are requesting a duplicate of, whether it is:

  • Your license plate.
  • Your license decal.
  • Your registration receipt.

After you have filled out all the appropriate sections of the car registration replacement application, you will then have the option to submit it to the Alabama Department of Revenue in one of two ways: in person or by mail. Take note that your car titling and registration office will only accept payment in the form of a check, money order, check, Visa, Mastercard.
