Request to Change Child Support

If you would like to change the amount of child support you receive for your child in the state of Alabama, you are required to fill out a Request to Change Child Support form and submit it to the District Court of the Alabama county you reside in.

How to Complete a Request to Change Child Support Form

In order to complete a Request to Change Child Support form in Alabama, you are required to provide the following information:

• Provide your information and the plaintiff’s information:
  • Full name
  • Address
  • Date of birth
  • Phone number
• List the date and place your current child support order was made.
• List the names of the children the child support covers.
• Explain the reason you are asking for a change in child support.
• State your request for the change in child support.
• Sign and date the form.

Additionally, you will have to fill out a Financial Declaration form. Once you have completed the required forms, take the originals along with two copies of each form to the District Court of the Alabama County you reside in. You will be required to pay a fee for filing each form with cash or a money order. If you are unable to pay the fee, you will be required to fill out a Affidavit of Substantial Hardship form and sign it in the presence of a notary.
